When you click the link below, you will be prompted to complete the patient's identifying information. Complete all required fields and provide a good contact number and email address where you can be reached. After you save the entered information, you will need to select and complete in its entirety the New Patient Packet relevant to the patient's age and services being sought. You can also find copies of these documents on our Forms page. After you have completed the appropriate packet, please submit all required documents to our office via email at contactus@jonesccta.com or fax at 817-453-2414.
Additional Important Information
- If custody documents or a divorce decree exist regarding the legal rights of a minor patient's parents/guardians to consent to and participate in treatment, those MUST be provided to our office prior to the onset of services. Failure to do so will prevent us from being able to treat your child.
- If you are late for your initial appointment and/or fail to complete the required documents in their entireties by your scheduled start time, your session will be cancelled and a $65 fee will be assessed.
